Family Intake, Assessment and Referral

CAASTLC conducts Intake, Assessment and Referral for all household members to determine eligibility and appropriate needs.

All persons are screened for income and residency eligibility requirements, after which appropriate needs are determined in order to design service delivery.  Referrals to appropriate services are then made and follow-up is conducted to determine if the services were rendered.

Required documentation:

  • Social security documentation for all household members over one (1) year of age.
  • Photo identification for the head of the household.
  • Proof of residency (must be living in St. Louis County).
