Family Support Case ManagementCAASTLC offers home-based Case Management services designed to assist the participant in becoming self-sufficient through employment and/or educational goals. The role of the Case Manager is to be a motivational and supportive role model and an advocate as needed. Areas that the Case Manager will explore with the participant include employment and education, budgeting, nutrition, household management, parenting, personal growth and development, literacy guidance, and referrals to other community resources as needed in order to move the client towards self-sufficiency. In order that the client continue to focus on employment and/or educational goals, crisis intervention assistance is provided as needed which may include energy assistance, food, housing assistance, or family counseling assistance. Eligibility Requirements - Household income may not exceed 125% of the federal poverty guidelines. Please go to our "Income Guidelines" page for a complete listing of income guidelines.
