Healthy HomesOur Healthy Homes program provides patients, recently discharged from the hospital, the opportunity to recover in a healthy environment. The patients included in our Healthy Homes program have been recently discharged from the hospital with treatment for respiratory issues, pneumonia, or asthma. Currently, 20% of Medicare patients discharged are readmitted to the hospital within 30 days. A healthy indoor air environment is critical for expedient recovery and to reduce avoidable readmissions. CAASTLC and North East Community Action Corporation (NECAC) are partnering with local hospitals in a pilot study to reduce the number of near and long term hospital readmissions for recently discharged patients through an in-home process of behavioral modifications and indoor environmental improvements. CAASLTC and NECAC use case managers to provide Healthy Homes education and address other identified family issues. Our Weatherization program will improve the indoor air quality through installing insulation, proper ventilation, air leakage control, and furnace repair or replacement. The Healthy Homes project is a pilot program involving about 30 patients with expectations that the program will be expanded. |
